What to Expect in Your DUI Evaluation

In Illinois, a DUI evaluation is mandatory following a DUI arrest—whether for sentencing purposes or to regain restricted or full driving privileges.

 

Evaluator Reviews:

  • An in-depth interview covering your substance use history, legal and driving history, and any prior treatment or evaluations .
  • Objective Test score, blood alcohol content (BAC) results, and a Court Purposes Driving Abstract .
  • A Law Enforcement Sworn Report or equivalent documentation .

 

State Regulatory Framework:

  • A structured face-to-face evaluation with informed consent documentation, collection of comprehensive substance-use history, and risk classification (Minimal, Moderate, Significant, or High) .
  • Evaluators follow the Illinois Administrative Code (Title 77, § 2060.503), ensuring every step meets legislative requirements concerning history collection, documentation, delivery of Uniform Report forms, and data confidentiality .

 

Risk Determination & Recommendations:

  • Minimal Risk → Minimum of 10 hours of DUI Risk Education
  • Moderate Risk → 10 hours of education + 12 hours of early intervention (structured over ≥4 weeks, ≤3 hrs/day), plus any needed treatment and continuing care  .
  • Significant Risk → 10 hours education + 20 hours of treatment + continuing care plan .
  • High Risk → 75 hours of substance abuse treatment + continuing care .

 

What You Need to Bring

  1. Government-issued photo ID (e.g., driver’s license)  
  2. Court Purposes Driving Abstract, obtained from the Secretary of State’s office—not an online printout .
  3. Law Enforcement Sworn Report or official documentation from the arresting officer .
  4. Chemical test results, such as BAC results or refusal documentation (often included in reports above).

 

Program Details & Fees

  • Evaluation Cost: $275
  • Deposit Required at Scheduling: $75 (applied toward evaluation cost)
  • Licensed & Certified: All evaluations conducted by SUPR-licensed professionals, following IDHS/DHS guidelines and Illinois regulations  .
