What is the D.U.I. and Substance Abuse Program?
♦ DUI Evaluations:
Professional evaluations for clients which include a full alcohol history and driving history.
♦ Risk Education Classes:
Ten (10) hours of required education. This class covers all required subjects as indicated by the State (State laws and consequences of drinking/substance abuse and driving, facts about alcoholism and other addictions, as well as a personal plan for change.
♦ D.U.I. Requirements:
Minimum Risk: 10 hours of Risk Education.
Moderate Risk: 12 hours of Early Intervention and 10 hours of Risk Education.
Significant Risk: 20 hours Substance Abuse treatment followed by aftercare and 10 hours of Risk Education.
High Risk: 75 hours of Substance Abuse Treatment followed by aftercare.
♦ All treatment includes an Intake to assure that we’re meeting client needs, group therapy, and an exit interview which includes a DUI prevention and continuing care plan.
♦ Non-D.U.I. Client Services:
♦ Court Ordered and Non-Court Ordered Evaluations to assess client needs.
♦ Outpatient Intervention treatment and Intensive Outpatient Treatment.
♦ Individual and group Outpatient Treatment includes:
Learning new coping skills and strategies.
Anger Management.
Education.
Relationship / Family issues.
Developing new social skills.
Difference of use, abuse and dependence.
Dealing with denial.
Interpersonal skills.
Relapse triggers.
Physical affects of alcohol/substance abuse dependency.
Addiction as a disease.

D.U.I. and Substance Abuse Program Clinicians
Kristie Polk, B.A., CADC
Kristie Polk, BA, CADC, is a Certified Alcohol and Drug Counselor and Certified Anger Management Specialist/Trainer. She received her addiction counseling certification from Waubonsee Community College and her BA in Applied Behavioral Sciences from National-Louis University. She provides education, early intervention and treatment services for adolescents and adults. She joined Crossroads Counseling Services in Sept, 2008.

Karen Krolak, LCSW, CADC
Karen Krolak is a Licensed Clinical Social Worker & Certified Alcohol and Drug Counselor. She earned her BS in Criminal Justice from Bemidji State University in 1992 & her Master of Social Work from the University of IL. in 2002. Her experience includes working with adults & families in the area of substance abuse. She joined Crossroads Counseling Services in May, 2010.

Linda Weyer, CADC
Linda Weyer, CADC, is a Certified Alcohol and Drug Counselor. Her substance abuse treatment experience includes working with adolescents and adults who are chemically dependent. in both inpatient and outpatient substance abuse facilities. In 2004, Linda
completed the DUI Service Provider Orientation, enabling her to conduct DUI Evaluations, Alcohol and Drug Evaluations, Secretary of State Evaluations and Risk Education, as well as intervention and treatment services for adults and adolescents. She joined Crossroads Counseling Services in February, 2012.
